Student Satisfaction Survey

Survey Period is April 13-May 22, 2015. Note: Requirement for Enrollment Clearance

The annual student satisfaction survey starts on April and ends on May 22, 2015.  In view of this, please accomplish the survey within the said period.

Once completed, a unique 15-digit reference number will be generated.  This number should then be sent to ustgssss@yahoo.com and will be used to verify if the students have already completed the survey.

Answering the survey is a requirement for enrollment clearance.
