APPLICATION AND ISSUANCE OF CAR STICKERS FOR AY 2016-2017

Please be informed that the application for UST Car Stickers for the Academic Year 2016-2017 shall start on Wednesday, 12 October 2016 up to November 15, 2016.

For your guidance and reference, please find below the requirements, schedule and venue of issuance, to wit;

General Requirements:

  1. Photocopy of UST ID/USTH ID
  2. Clear Photocopies of OR/CR of vehicle
  3. One (1) copy of latest 2×2  ID picture
  4. Deed of Sale (if applicable only)
  5. Please bring vehicle at the Place of Issuance

Additional Requirements for Students:

  1. Photocopy of Student Registration
  2. One (1) copy of latest 2×2 ID picture of Student

FOR ADMINISTRATIVE/ACADEMIC OFFICIALS, FACULTY MEMBERS AND SUPPORT STAFF

  • October 12, 2016 (Venue) QUADRICENTENNIAL PAVILION

FOR STUDENTS

  • October 20-27, 2016  and November 7-15, 2016 (Venue) QUADRICENTENNIAL PAVILION

Strict Implementation of car stickers shall start on Wednesday, November 16, 2016.
